Toggle ringtone mute when headphones dis/connected

If you're not a monster, you probably keep your phone on silent/vibrate virtually all the time. But what about when you put earbuds in and then get a call you can't hear? It's no fun.

This flow fixes that. When you connect an audio output device, it unmutes your ringtone so you'll hear calls; when you disconnect, it's muted again.

Related functionality that you can adjust or remove:
* Announces that the ringtone has been turned on via audio alert and toast message
* Announces that the ringtone has been turned off via toast message only
* Turns up the media volume when the device connects in case it wasn't already (does not change media volume at all when disconnecting)
